Eligible risks must have two years prior and continuous workers compensation coverage.

Two years of 60% or less loss ratio.
 |
 |
Eligible risks include: human
service risks, including residential, mental and retardation
facilities, vocational rehabilitation services and training,
information assistance services, autistic youth services,
nonresidential case management services, religious organizations,
Goodwill Industries, Industries for the Blind, ARC (Association
for Retarded Citizens) Services and drug or alcohol rehabilitation
or substance abuse facilities.

factors will be submitted to the carrier for approval. |
The following minimum premiums apply:

$5,000 for professional organizations.

$20,000 for Community Service
